Upcoming catalystThe event that could move the share price.
UnitedHealth Group will release third-quarter 2026 results before the US market opens on Tuesday 13 October 2026. The company has not published an exact release time. Its analyst call begins at 8:00 a.m. Eastern.
Main stock affectedThe company the event affects most directly.
UnitedHealth Group Incorporated, New York Stock Exchange ticker UNH.
Current priceWhere the shares traded when this write-up was made, and when that was.
$376.59 as of 25 Sept 2026, 20:00 UTC, the regular-session close.
Expected stock reactionWhich way we expect the share price to go after the event. Positive means it rises, Negative means it falls, Mixed means it could go either way.
Positive. The outcome work puts cost improvement with guidance above $20.00 at 45%, a maintained or narrowed outlook at 30% and renewed cost pressure at 25%. The upside is the likeliest single outcome by a wide margin, and it is where the weight of the last two quarters points. The case against it is the bar. The full-year medical care ratio is already guided to 88.1% plus or minus 25 basis points, tightened from 88.8% plus or minus 50, and adjusted earnings to $19.50 to $20.00. Asking for a third consecutive raise against a band that narrow is demanding, and sell-side work into the print expects results that merely meet or slightly top consensus. The downside is the bigger move, 10% to 18% against 8% to 12%, so a one in four chance still carries real weight. That is what the risk rating and the figure below are for, not the direction.
Action before the eventWhat we suggest you do before the event. Buy, Watch or Avoid.
Watch. The turnaround is real enough to respect, but the options market prices a large move and the last four earnings reactions ranged from a 19.61% fall to a 6.96% gain. Let the medical-cost evidence and guidance decide the trade.
Why the stock is affectedThe link between the event and this company.
UnitedHealth earns insurance premiums through UnitedHealthcare and fees across Optum's care delivery, pharmacy benefits and technology businesses. Medical costs consume most insurance premiums, so a small change in the medical care ratio can move billions of dollars of profit. The report will also show whether Optum Health is stabilizing and whether management's restructuring is producing sustainable earnings rather than a one-quarter benefit.
Catalyst confidenceHow sure we are that the event happens as described. Scored out of 100. Higher means more sure.
The event is confirmed and has 97% confidence of occurring as scheduled. My likely outcome is another earnings beat with full-year guidance maintained or raised modestly. Confidence is lower on the stock direction because expectations have recovered and second-quarter earnings benefited from favorable prior-period reserve development.
Stock-reaction confidenceHow sure we are that the stock reacts the way we expect. Scored out of 100. Higher means more sure.
56%. UNH has become unusually volatile around earnings and the options market prices about 8.7% either way, so the stock will move. The direction rests on the medical cost line confirming what the last two quarters showed, and one of the things that could have gone wrong before the print has now gone right: the star ratings published on 1 October left more than 77% of members in four-star or better plans, roughly flat, while Humana and Aetna slipped. The honest risk is still that favourable reserve development flatters a quarter that is deteriorating underneath, which is how this call goes right in the first hour and wrong in the second.
When the market will reactThe moment the price is likely to move, such as at the announcement or at the next market open.
The first move should occur in premarket trading after the release. The 8:00 a.m. Eastern call can produce a second move when management explains utilization, pricing, reserve development, Optum Health and 2027. The cleanest conclusion should form between the call and the close of the following session.
Effect on the businessWhat the event changes for the company itself, such as its sales, costs or profit.
High. Full-year adjusted earnings guidance of $19.50 to $20.00, a medical care ratio of 88.1% plus or minus 25 basis points and operating cash flow near $24 billion all depend on continued cost discipline. A better cost ratio raises insurance margin immediately. A worse ratio means pricing still has not caught up with care use and can reduce both current profit and confidence in 2027.
Market expectations and forecastWhat investors already expect from this event, and the numbers being forecast.
Published consensus is approximately $4.17 to $4.19 of adjusted earnings per share and $112.5 billion of revenue. The year-earlier quarter produced about $2.92 of adjusted earnings per share and $113.2 billion of revenue during the reset. The low earnings comparison makes growth likely even if revenue is nearly flat. My forecast is adjusted earnings of $4.35 to $4.60 a share on $112 billion to $114 billion of revenue. I expect the medical care ratio near 88%, within guidance, and full-year adjusted earnings maintained or narrowed toward the upper half. That would confirm recovery but may not be enough for a large gain. A stronger trade needs a lower cost ratio, better Optum Health profitability and guidance above $20.00.
How much is already priced inHow much of the expected outcome the share price already reflects before the event happens.
Medium. The second quarter beat consensus by $1.44 a share and management raised adjusted guidance from more than $17.75 to $19.50 through $20.00. The shares are still 18.4% below the July high and below the 50-day average, so investors have not priced a complete return to the old earnings trajectory. They have priced meaningful recovery, making the quality of medical-cost improvement more important than another routine beat.
What else lands that dayOther news and events on the same day that could push the price around, and whether they help this trade, bury it, or work against it.
JPMorgan, Citigroup and Wells Fargo also report that morning, making 13 October one of the quarter's busiest financial-results sessions. Those banks do not determine UNH's medical costs, but their size can dominate index futures and the opening tape. No major Bureau of Labor Statistics release is scheduled that morning. The day is crowded. A premarket UNH move can be diluted by broad index activity, but a move that persists after the 8:00 a.m. call is more likely to reflect company evidence. Traders should avoid reading the first thin premarket print as the final verdict.
Expected price move and durationHow far the share price may move because of this event, and how long that move may last.
Options-Implied Move The options market prices approximately ±8.7% through the 16 October expiration. The plus or minus sign describes size, not direction. At $376.59, that is about $32.77 and an approximate $343.82 to $409.36 range. The contract includes three post-report sessions and broad market activity, so it is not a pure one-day estimate. Historical Event Analysis Second-quarter 2026 results on 16 July produced a 1.16% close-to-close gain. Adjusted earnings beat by roughly $1.44 and guidance rose, but the stock surrendered most of a much larger intraday gain. First-quarter 2026 results on 21 April produced a 6.96% close-to-close gain. Results beat, cost trends improved and the market treated the report as confirmation that the reset was working. Fourth-quarter 2025 results on 27 January 2026 produced a 19.61% close-to-close loss. The initial 2026 outlook fell far below the earnings power investors once expected and exposed the scale of the margin reset. Third-quarter 2025 results on 28 October produced a 0.51% close-to-close gain. Results and guidance improved, but an 89.9% medical care ratio kept enthusiasm contained. Historical Average Move The four reactions went both ways. Their average absolute move was 1.16% plus 6.96% plus 19.61% plus 0.51%, divided by four, or ±7.06%. The sample is small but directly comparable and shows a much hotter earnings regime than UNH's old defensive reputation suggests. Our Expected Move My estimate is +8% to +12% if the cost ratio beats guidance and adjusted earnings guidance rises above $20.00, and -10% to -18% if utilization worsens or management signals the recovery is delayed. A result close to plan should trade between -4% and +4%. The asymmetric downside reflects the January collapse and the risk that favorable reserve development has made recent earnings look cleaner than underlying current-period costs. Impact Strength High. A change of only one percentage point in a premium base measured in hundreds of billions can materially alter profit. The report also tests whether the company can restore forecasting credibility after the 2025 reset. Estimated Duration One to three trading days for an ordinary beat. A medical-cost surprise or material 2027 signal could reprice the stock for several weeks because it changes the earnings base rather than only one quarter.
Result needed for a big moveWhat the outcome has to be for the stock to move a lot instead of a little.
A strong gain likely requires adjusted earnings above about $4.60, a medical care ratio below 87.8%, better Optum Health margin and adjusted guidance above $20.00. A large decline becomes likely if the cost ratio exceeds about 88.5%, current-period utilization worsens or management lowers the $19.50 to $20.00 range. These thresholds are my estimates, not company targets.
Possible outcomesThe main ways the event could turn out, with the expected stock reaction for each one.
Positive outcome, 45% probability. Medical costs improve on current-period experience, Optum Health stabilizes and guidance moves above $20.00. Expected reaction: up 8% to 12%. Middle outcome, 30% probability. Earnings beat the low comparison, the cost ratio stays near 88% and guidance is maintained or narrowed. Expected reaction: down 4% to up 4%. Negative outcome, 25% probability. Utilization, reimbursement or Optum Health costs worsen and guidance is reduced or left unchanged with cautious language. Expected reaction: down 10% to 18%.
Other affected stocksOther companies that could move on the same news, such as suppliers and competitors.
Elevance Health, Humana and CVS Health can react to the Medicare Advantage and medical-utilization read-through. Centene is sensitive to Medicaid and exchange trends. Cigna can move on Optum Rx commentary because pharmacy-benefit economics affect the entire group.
What the market may be missingSomething we think investors have overlooked or read wrong.
The second-quarter medical care ratio was 86.7%, but it included $860 million of favorable prior-period medical development. That means some apparent improvement came from updating estimates for claims incurred earlier, not only from lower current care use. Third-quarter current-period experience is therefore more valuable than the headline ratio alone. The market may also be underestimating how seasonal the recovery is. Management said UnitedHealthcare and Optum Health earnings are weighted to the first half, with Optum Health expected to earn only a modest profit in the third quarter and potentially lose money in the fourth. A sequential profit decline can be consistent with guidance and still disappoint investors focused on the second-quarter run rate.
Main risksWhat could go wrong, and what would prove this call wrong.
Overall event risk is High. Medical utilization can change faster than pricing. Medicare Advantage reimbursement and risk adjustment still affect revenue, though star ratings are now a known quantity rather than an open one: the ratings published on 1 October held more than 77% of members in four-star or better plans, with 40% at 4.5 stars, roughly flat on the year. Federal civil and criminal investigations into Medicare practices create legal and reputational uncertainty. Optum and UnitedHealthcare transactions also face antitrust scrutiny. A clean quarter does not remove those risks.
Other factorsAnything else worth knowing that the sections above do not cover.
The full-year plan assumes at least $5 billion of repurchases and about $24 billion of operating cash flow. Those figures provide support but cannot compensate for a renewed medical-cost miss. The most useful call questions are whether favorable reserve development repeated, how current Medicare trends compare with pricing and whether management has enough visibility to discuss 2027.
Trend over 6 to 12 monthsWhich way the share price has been heading over the past year, and how far it has travelled.
UNH gained 40.49% over six months and 7.04% over twelve months through 25 September, but that hides a violent fall and rebound. The 52-week range is $255.97 in March to $461.62 in July. The current price is 18.42% below the high and 47.12% above the low.
Recent price actionWhat the shares have done over the last few weeks and sessions, and on what kind of volume.
The stock lost 6.09% in one month and 11.99% in three months. It fell from $399.26 intraday on 10 September to $375.90 the next day, then based between roughly $366 and $381 through 25 September. Volume rose during the decline, including 7.48 million shares on 11 September and 8.62 million on 18 September. Price is 58.7% of the way from its 52-week low to its high.
Market structureThe shape of the chart. Rising highs and rising lows mean an uptrend, falling ones mean a downtrend, and neither means the price is stuck in a range.
The six-month chart has a higher low and a recovery from the March trough, while the three-month chart has lower highs from July. That is a broad recovery with a short-term correction. Buyers have defended the high $360s, but rallies have struggled near $400.
Support and resistance zonesPrice areas where buyers have stepped in before, and price areas where sellers have. They often slow the price down when it gets there again.
First support is $366 to $372, tested on 18, 22, 23 and 24 September. A close below $366 would weaken the recovery and expose $350 to $355 around the 200-day average. First resistance is $388 to $400, tested several times in September and containing the 50-day average at $399.38. A close above $400 would improve momentum. Stronger resistance is $423 to $430 around the post-earnings July area, followed by the $461.62 high.
50-day and 200-day moving averagesThe average closing price over the last 50 days and the last 200 days. Where the price sits against them shows whether the recent trend is strong or fading.
A moving average is the average close over the named number of sessions. The 50-day average is $399.38 and is flattening after a sharp rise. Price is 5.71% below it. The 200-day average is $355.43 and is turning higher. Price is 5.95% above it. The 50-day remains $43.95 above the 200-day, a bullish longer-term arrangement, but losing the 50-day confirms a short-term correction.
Relative strengthHow this stock has performed against the wider market and against its own sector over the same period.
UNH gained 7.04% over twelve months against 16.68% for the S&P 500 tracking fund and 25.16% for the Health Care Select Sector fund. It lagged them by 9.64 and 18.12 percentage points. Over six months, UNH gained 40.49%, beating the S&P 500 by 20.92 points and XLV by 23.36. In the latest month it lagged XLV by 4.45 points, so the powerful rebound has lost leadership recently.
Volume trendHow many shares are changing hands lately compared with normal. Heavy volume behind a move means more traders agree with it.
Average volume over 20 sessions is 5.12 million shares against 4.79 million over 50 sessions, about 6.96% higher. Rising activity during a pullback confirms active selling and repositioning, though it is far below the panic volume seen during the 2025 crisis.
VolatilityHow much the share price swings day to day right now, compared with how much it usually swings.
The 14-day average true range is $9.99, or 2.65% of price. The average intraday range over 20 sessions is $8.98, or 2.38%. Annualised realised volatility is 18.23% over 14 sessions against 21.19% over 50, so day-to-day volatility has calmed but remains meaningful. Position size must tolerate a routine $10 daily range, while earnings risk is several times larger.
Long-Term Technical ViewThe wider chart over years. The big trend, the major price levels and how much the stock swings.
The multi-year uptrend broke during the 2025 earnings and leadership crisis, and the stock remains well below its former peak above $600. The recovery from $255.97 is substantial, but $423 to $462 is a large overhead-supply zone. The rising 200-day average near $355 is the key long-term support.
Technical biasWhat the chart alone suggests, ignoring the news. Rated Bullish, Neutral or Bearish.
Neutral. Price is above a rising 200-day average and the 50-day remains above it, but price has fallen below the 50-day and relative strength has weakened for three months. The neutral chart agrees with the mixed catalyst call.
What would invalidate this biasThe price level or chart change that would prove the technical read wrong.
A close above $400 on above-average volume would turn the bias Bullish and reopen $423 to $430. A close below $366 would turn it Bearish, with the 200-day average near $355 as the next test.

Stock OverviewThe basic numbers. Price, size of the company, sector, the past year of highs and lows, trading volume and the next earnings date.
UNH closed at $376.59 at 20:00 UTC on 25 September 2026. With 905 million shares outstanding at 30 June, approximate market value is $341 billion. It is a managed-healthcare and healthcare-services company. The 52-week range is $255.97 to $461.62 and average 20-day volume is 5.12 million shares. The next confirmed earnings date is 13 October 2026.
What the company doesThe business in plain words. What it sells, who buys it, and how the money reaches the company.
UnitedHealthcare collects premiums from employers, individuals and government programs and pays for members' medical care. Optum Health runs and supports clinics, Optum Insight sells healthcare technology and services, and Optum Rx manages pharmacy benefits and fills prescriptions. Revenue is recurring but not subscription-like profit because claims, drug costs and government reimbursement can change after prices are set.
Revenue BreakdownWhere the money comes from, split by product, by part of the business and by country.
Second-quarter consolidated revenue was $112.0 billion. UnitedHealthcare produced $87.3 billion before internal eliminations. Optum produced $65.7 billion before eliminations, including Optum Rx, Optum Health and Optum Insight. The two groups overlap because UnitedHealthcare buys services from Optum, so their revenue cannot be added to obtain the consolidated total. The United States supplies the great majority of revenue after the company reduced South American operations. Insurance premiums are the largest revenue source and medical costs are the largest expense.
Company GrowthHow sales, profit, earnings per share and spare cash have changed over the last few years.
Revenue rose from $371.6 billion in 2023 to $400.3 billion in 2024 and $447.6 billion in 2025, a two-year compound growth rate near 9.7%. Net income attributable to common shareholders fell from $22.4 billion to $14.4 billion and $12.1 billion. Diluted earnings per share fell from $23.86 to $15.51 and $13.23. Operating cash flow was $29.1 billion in 2023, $24.2 billion in 2024 and $19.7 billion in 2025. Revenue growth remained strong while profit and cash flow deteriorated, showing that medical costs and restructuring mattered more than sales volume. Current growth is Medium because 2026 earnings are recovering from a low base rather than compounding from the old trend.
Earnings PerformanceThe latest results, whether they beat or missed what the market expected, and what the company says comes next.
Second-quarter revenue of $112.0 billion beat consensus near $110.8 billion and adjusted earnings of $6.38 beat roughly $4.94. First-quarter adjusted earnings also beat and started the recovery. Fourth-quarter 2025 established a much lower 2026 base and the stock fell 19.61%. Third-quarter 2025 beat low expectations but the cost ratio remained high. Management now expects 2026 adjusted earnings of $19.50 to $20.00, reported earnings of $18.45 to $18.95, a medical care ratio of 88.1% plus or minus 25 basis points, operating cash flow near $24 billion and at least $5 billion of repurchases. The adjusted earnings, cash flow and buyback guidance were raised in July.
Financial HealthCash, debt, cash flow and profit margins, and whether the company keeps issuing new shares. Rated Strong, Average or Weak.
At 30 June, cash was $28.6 billion, available-for-sale debt securities were $50.3 billion and marketable equity securities were $2.1 billion. Short-term borrowings and current debt were $3.8 billion and long-term debt was $69.5 billion. Net debt against cash alone was about $44.7 billion. Operating cash flow was $20.0 billion in the first half and capital spending was $1.6 billion, producing roughly $18.4 billion of cash flow after capital spending. The second-quarter net margin was 4.9%, while the insurance medical care ratio was 86.7%. The company does not report a useful conventional gross margin because medical claims are its core cost. Diluted shares fell from 929 million in 2024 to 911 million in 2025 and stood near 905 million in June 2026. Liquidity is strong enough to withstand a bad year, but $73.3 billion of total debt and negative rating outlooks require discipline.
ValuationWhat you pay today for what the company earns, next to its rivals and its own past. Rated Cheap, Fair or Expensive.
At $376.59 and trailing earnings near $15.54 a share, the price-to-earnings ratio is about 24.2. Against the midpoint of 2026 adjusted guidance, the multiple is about 19.0. Enterprise value to EBITDA is less informative for an insurer because reserves and investment assets are operating items. A PEG ratio is unstable while earnings rebound from a depressed base. The stock historically earned a premium for consistent high-teens growth, but that record was broken. The current forward multiple is below the old premium yet above a distressed insurer valuation. Valuation is Fair because it balances recovery potential against medical-cost and legal uncertainty.
ShareholdersWho owns the company. Big funds, company insiders, and whether ownership sits with only a few holders.
The 2026 proxy reported Vanguard at about 10.11% before an internal ownership realignment and BlackRock at about 7.99%. Directors and executives own far less than 1% as a group. Ownership is broad and institutional, so no founder controls the company. Large index-manager positions mainly affect flows rather than operating decisions.
Insider ActivityWhether the people running the company have been buying or selling their own shares lately.
Stephen Hemsley bought 86,700 shares in the open market at $288.57 on 16 May 2025, a personal investment of about $25.0 million. That is a meaningful discretionary purchase rather than compensation. More recently, Optum chief Patrick Conway sold 1,169 shares at $390 on 25 August 2026. The older CEO purchase remains supportive, but one well-timed buy does not resolve current medical-cost or legal risks.
Dividends and BuybacksCash paid out to shareholders, whether that payment looks safe, and whether the company is buying back its own shares.
The annual dividend rate is $9.28, or about 2.46% at the current price, after another increase in 2026. UnitedHealth has paid and regularly raised its dividend for many years. The annual rate is roughly 47% of the midpoint of adjusted earnings guidance and appears covered. Management lifted the 2026 repurchase target to at least $5 billion. First-half repurchases were $1.65 billion, leaving execution weighted to the second half.
Competitive PositionWho the company competes with, how much of the market it holds, and what it has that rivals do not.
UnitedHealth combines a national insurance franchise with care delivery, pharmacy benefits, data and payments. Elevance, Humana, CVS Health, Cigna and Centene compete in parts of that chain, but none matches the same breadth at comparable scale. Integration creates data, purchasing and cross-selling advantages. It also creates the central vulnerability because regulators, customers and providers can view that same vertical reach as a conflict or concentration of power.
Future OutlookWhat sales and profit are expected to do next, and what management has promised.
Management expects 2026 adjusted earnings of $19.50 to $20.00 and operating earnings above $25.45 billion. Published estimates put 2027 earnings near $22.54 a share, about 13.5% growth. My outlook is Positive but fragile. Better pricing and restructuring can restore earnings growth, while the old predictability will require several quarters of cost ratios that do not rely on favorable reserve development.
Other Upcoming Company CatalystsOther events ahead that could move the share price, such as results, launches or approvals, apart from the catalyst this report is built on.
Medicare Advantage star-rating updates, 2027 pricing and annual enrollment can change membership and reimbursement into year-end. Federal investigations and antitrust cases can produce unscheduled disclosures. Fourth-quarter results in January 2027 should provide the first formal full-year 2027 outlook. Exact dates for legal decisions are not public.
Long-Term Company RisksWhat could weaken the business over the coming years, rather than around this one event.
Persistent medical-cost inflation is the largest operating risk because prices are often set before claims are known. Medicare funding, risk-adjustment audits and star ratings can reduce revenue. Federal civil and criminal investigations create fines, business-practice and reputation risk. Optum's vertical integration faces political and antitrust pressure. Cybersecurity remains material after the Change Healthcare attack. Debt is manageable but reduces flexibility if cash generation disappoints.
Traderavel VerdictOur straight answer on the company after everything above, not on the trade. Quality, growth, price, risk and the long-term view.
UnitedHealth remains a strong-quality company with medium growth, strong financial health, a fair valuation and medium long-term company risk. Its scale and integrated assets support a Positive outlook, but forecasting credibility is still being rebuilt. It suits long-term healthcare investors willing to monitor regulation and medical-cost cycles. It does not suit investors seeking a predictable defensive stock or low event volatility.
