Upcoming catalystThe event that could move the share price.
The FDA target action date for Merck’s Winrevair label update based on the Phase 3 HYPERION trial is September 21, 2026. No exact announcement time is confirmed.
Main stock affectedThe company the event affects most directly.
Merck & Co., Inc., ticker MRK.
Current priceWhere the shares traded when this write-up was made, and when that was.
$152.55 as of 21 Aug 2026, 20:00 UTC, the regular-session close
Expected stock reactionWhich way we expect the share price to go after the event. Positive means it rises, Negative means it falls, Mixed means it could go either way.
Positive, but likely modest. Approval should support confidence in Winrevair’s long-term growth. The immediate upside is limited because the drug is already approved and the stock recently reached a 52-week high.
Action before the eventWhat we suggest you do before the event. Buy, Watch or Avoid.
Watch. Do not chase MRK near its recent high. The better setup is a pullback before the event, or a clean approval with stronger-than-expected label language. The approval probability looks high, but the reward compared with the current price risk is limited.
Why the stock is affectedThe link between the event and this company.
HYPERION studied Winrevair in recently diagnosed pulmonary arterial hypertension patients. Winrevair reduced the risk of clinical worsening by 76%, with a hazard ratio of 0.24 and a p-value below 0.0001. A favorable label update could encourage doctors to use Winrevair earlier in treatment, increasing the number of patients treated and the average time each patient remains on therapy.
Catalyst confidenceHow sure we are that the event happens as described. Scored out of 100. Higher means more sure.
Likely outcome: FDA approval of the HYPERION label update. Confidence: 93 out of 100. The drug is already approved, the Phase 3 trial produced strong results, and Merck has reported no confirmed clinical or regulatory problem that would clearly block the update.
Stock-reaction confidenceHow sure we are that the stock reacts the way we expect. Scored out of 100. Higher means more sure.
Confidence: 61 out of 100 that MRK trades higher. The business outcome is easier to forecast than the share-price reaction. MRK has already rallied sharply on separate cancer-vaccine news, creating meaningful sell-the-news risk.
When the market will reactThe moment the price is likely to move, such as at the announcement or at the next market open.
The FDA may announce before September 21 because PDUFA dates are deadlines, not promised announcement times. If news arrives during market hours, MRK should react immediately. If it arrives after the close or over the weekend, the main reaction should occur at the next regular-session open. The clearest trading window is the announcement through the following one to three trading days.
Effect on the businessWhat the event changes for the company itself, such as its sales, costs or profit.
A clean approval should strengthen Winrevair’s position for earlier use in pulmonary arterial hypertension. Winrevair generated $588 million of second-quarter 2026 sales, up 75% year over year. That equals a simple annualized revenue pace of about $2.35 billion, calculated as $588 million multiplied by four. This is not a formal forecast because quarterly sales can change. The update may support patient growth and longer treatment duration. However, it does not create a completely new disease market. This makes the commercial effect important over time but less powerful as a one-day catalyst. Merck pays Bristol Myers Squibb a 22% royalty on Winrevair net sales. This reduces Merck’s product margin but gives BMY a secondary benefit from stronger adoption.
Market expectations and forecastWhat investors already expect from this event, and the numbers being forecast.
Expected: Approval is the clear base case because HYPERION met its primary endpoint with a 76% reduction in clinical-worsening risk. My forecast: The FDA approves an update that reinforces Winrevair use in recently diagnosed adults with pulmonary arterial hypertension. I estimate a 93% approval probability. Trading interpretation: A routine approval may produce only a small gain. A stronger label that clearly supports earlier treatment could produce a larger move. A delay, unexpected restriction or new safety language would have a larger negative effect because expectations are favorable. Recent price action and priced-in level Priced-in level: High. MRK rose from $126.26 on July 21 to $152.55 on August 21. The gain was $26.29 per share, or 20.8%, calculated as $26.29 divided by $126.26. The stock jumped 12.6% on August 19 after positive Phase 3 melanoma cancer-vaccine results with Moderna. Volume reached about 32.8 million shares that day, compared with roughly 6 million to 11 million shares on many recent sessions. MRK then reached a new 52-week high of $154.49 on August 21. This rally was driven mainly by oncology news, not Winrevair. Even so, the higher share price makes another near-term rally harder because investors now require stronger evidence to justify additional gains.
Expected price move and durationHow far the share price may move because of this event, and how long that move may last.
Options-Implied Move The September 25, 2026 $152.50 call had a bid of $5.15 and ask of $6.10. The matching put had a bid of $5.05 and ask of $5.90. Call midpoint: $5.625. Put midpoint: $5.475. Combined straddle: $11.10. Implied move calculation: $11.10 divided by $152.55 equals 7.28%. Options-implied move: ±7.3% through September 25, 2026. The ± figure measures possible move size, not direction. It does not mean the options market agrees or disagrees with my positive call. This is only moderately reliable. The bid-ask spreads are wide and the options cover a full month of risks, not only the FDA decision. Historical Event Analysis March 26, 2024: The FDA granted the original Winrevair approval. MRK gained about 4.8% in the initial after-hours reaction. The following regular session showed a gain of roughly 4.4% during morning trading. October 27, 2025: The FDA approved the ZENITH label update. This is highly comparable, but I cannot confirm a clean stock move caused only by that approval. It is excluded from the calculation rather than estimated. Historical Average Move Historical average move: +4.8%. Calculation: One verified comparable event with a +4.8% initial reaction. The sample is limited, so this should be treated as a reference, not a dependable average. Our Expected Move Our expected move: +0.5% to +2.0%. This is below the ±7.3% options-implied move and below the +4.8% historical reference. The upcoming event is a label update for an established product, while the 2024 comparison was the drug’s first approval. MRK’s recent 20.8% one-month rally also reduces the likely incremental reaction. Impact Strength Low. The decision matters for long-term Winrevair growth, but a routine approval should have a limited immediate effect on a company with a market value above $370 billion. Estimated Duration 1-3 trading days. A routine approval reaction should fade quickly. Stronger-than-expected label language could support a longer repricing over several weeks as analysts increase Winrevair sales forecasts.
Result needed for a big moveWhat the outcome has to be for the stock to move a lot instead of a little.
For a gain above 3%, Merck likely needs more than basic approval. The label would need to clearly support earlier treatment, broad use in recently diagnosed patients and no meaningful new safety restriction. A significant negative move would require a delay, complete response letter, unexpected patient restriction or warning that reduces use.
Possible outcomesThe main ways the event could turn out, with the expected stock reaction for each one.
Positive scenario: The FDA approves broad HYPERION language with no important new warning. Expected reaction: +1% to +3%. Neutral scenario: The FDA approves the expected wording, but the update adds little beyond information doctors already know. Expected reaction: -0.5% to +1%. Negative scenario: The FDA delays the decision, requests more information or adds restrictive safety language. Expected reaction: -2% to -5%.
Other affected stocksOther companies that could move on the same news, such as suppliers and competitors.
BMY: Positive. Bristol Myers Squibb receives a 22% royalty on Winrevair net sales. UTHR: Mildly negative. Earlier Winrevair adoption could increase competitive pressure on established pulmonary arterial hypertension treatments. JNJ: Mildly negative. Johnson & Johnson also sells pulmonary arterial hypertension therapies and could face greater treatment-share pressure.
What the market may be missingSomething we think investors have overlooked or read wrong.
The main opportunity is treatment timing. The label update may not add a completely new patient population, but it could shift Winrevair closer to the start of treatment. Simple example: If a patient begins Winrevair one year earlier and remains on it for the same total disease period, Merck receives an extra year of revenue from that patient. Earlier use can therefore increase lifetime revenue without changing the number of diagnosed patients. The overlooked risk is that doctors may already be using the published HYPERION evidence in treatment decisions. If so, formal label approval may change prescribing more slowly than investors expect.
What else lands that dayOther news and events on the same day that could push the price around, and whether they help this trade, bury it, or work against it.
21 September 2026 has an almost empty macro calendar. The only scheduled item is a New York Fed household spending survey at 15:00 UTC, which does not move markets. What the day does carry is the effective date of the September S&P index changes, which take effect before the open following the 18 September rebalance, so the opening auction on a large-capitalisation index constituent like Merck carries mechanical flow having nothing to do with a label update. No inflation release, no Federal Reserve event and no jobs data compete for attention. Net effect: this is one of the cleanest days on the calendar for a company-specific decision to be read on its own merits, with a distorted opening print as the only caveat. What to expect on the day. A PDUFA date is a deadline rather than an appointment, so the decision may land at any hour or slip. If it lands during the session, expect an unusually clean reaction, because there is genuinely nothing else for the market to trade. The one thing not to do is measure the move from the opening print, because rebalancing flow sets that price. A label expansion on an approved drug is in any case a slow re-rating on prescription evidence rather than a one-session repricing.
Main risksWhat could go wrong, and what would prove this call wrong.
Overall risk level: Medium. The main risks are a sell-the-news reaction, unexpected FDA safety language, limited commercial change from the new wording and a broader market decline. The recent cancer-vaccine rally has also increased MRK’s valuation and short-term profit-taking risk. That separate news may dominate trading more than the Winrevair decision.
Other factorsAnything else worth knowing that the sections above do not cover.
Merck’s August 19 melanoma cancer-vaccine result is currently the stronger stock-price driver. Any new cancer-vaccine details, analyst forecast changes or profit-taking could overwhelm the Winrevair reaction. The September 25 options also include market and company risks outside this FDA event. You should not treat the ±7.3% figure as a pure Winrevair forecast.
