FDA decision on Ziihera combination treatments for first-line HER2-positive, unresectable locally advanced or metastatic gastroesophageal adenocarcinoma. The PDUFA target date is August 25, 2026. The FDA has not published an exact decision time.

Positive.
Action before the eventWhat we suggest you do before the event. Buy, Watch or Avoid.
Watch. Do not chase expensive short-term options before a binary FDA decision. My preferred trade is to buy after approval if the label is broad and the stock opens less than 6% higher. This reduces rejection risk while preserving exposure to a multi-quarter commercial launch.
Why the stock is affectedThe link between the event and this company.
Ziihera currently produces limited revenue from its smaller biliary tract cancer indication. It generated $15 million of sales in the second quarter of 2026. Approval in first-line gastroesophageal cancer would give Jazz access to a larger patient population and allow an immediate US launch. About 20% of gastroesophageal adenocarcinoma patients have HER2-positive disease. The Phase 3 study showed both Ziihera regimens reduced the risk of progression or death by about 35% versus trastuzumab plus chemotherapy. The regimen containing Ziihera, tislelizumab and chemotherapy produced median overall survival of 26.4 months, more than seven months longer than the control arm. These results support approval and could change prescribing decisions.
Catalyst confidenceHow sure we are that the event happens as described. Scored out of 100. Higher means more sure.
Approval is my base case, with 86% confidence. Priority Review, Breakthrough Therapy designation, Real-Time Oncology Review, strong Phase 3 progression-free survival results and statistically significant survival data for the three-drug regimen support this forecast.
Stock-reaction confidenceHow sure we are that the stock reacts the way we expect. Scored out of 100. Higher means more sure.
I have 67% confidence that JAZZ rises following a broad approval. Approval is expected, so label quality and management's launch outlook will matter more than the approval headline alone.
When the market will reactThe moment the price is likely to move, such as at the announcement or at the next market open.
The stock can react immediately if the decision is released before or during trading on August 25. If news arrives after the close, the main move should occur in after-hours trading and at the August 26 open. The first reaction should last one to three sessions. Analyst estimate changes could extend the move for several weeks.
Effect on the businessWhat the event changes for the company itself, such as its sales, costs or profit.
Ziihera would move from a small, later-line biliary cancer product toward a larger first-line cancer opportunity. Jazz has stated that it is prepared to launch immediately following approval. Second-quarter Ziihera sales were $15 million, equal to about 1.25% of Jazz's $1.2 billion quarterly revenue. The calculation is $15 million divided by $1.2 billion. A simple example shows the longer-term leverage. Each additional $100 million of annual Ziihera sales would equal about 2.1% of Jazz's current annualized quarterly revenue base of $4.8 billion. This example is not a sales forecast.
Market expectations and forecastWhat investors already expect from this event, and the numbers being forecast.
The market expects approval because the pivotal trial met its progression-free survival goals, the three-drug regimen showed a statistically significant survival benefit and the application received Priority Review. Jazz is already preparing for an immediate launch. My forecast is approval for at least the Ziihera, tislelizumab and chemotherapy regimen. I also expect approval of Ziihera plus chemotherapy, but confidence in that part of the label is lower because its overall survival result had not reached statistical significance at the first interim analysis. Trading forecast: broad approval should produce a 4% to 8% gain. Approval with a narrower label could produce a move between minus 2% and plus 3%. A delay or rejection could produce a 15% to 25% decline. Recent price action and priced-in level Priced-in level: medium. JAZZ increased from $250.41 on July 22 to $254.28 at the August 21 regular-session close. That is a 1.55% increase, calculated as $254.28 divided by $250.41, minus one. The stock has therefore not experienced a large one-month approval run-up. The stock closed at $225.92 on June 22, making the two-month increase about 12.55%. The calculation is $254.28 divided by $225.92, minus one. Part of that move followed strong second-quarter results and increased company revenue guidance, so it cannot all be assigned to Ziihera. Latest volume was about 517,000 shares. This was near the lower end of the recent normal range and far below the 1.89 million shares traded after the August earnings report. The evidence suggests investors expect approval, but there is no clear final surge into the event.
Expected price move and durationHow far the share price may move because of this event, and how long that move may last.
Expected event move: 4% to 8% on a broad approval. Impact: Medium. The strongest move should occur over one to three trading days, with potential follow-through over two to four weeks if analysts raise long-term Ziihera estimates. The options market indicated a 12.67% move over 29 days as of August 20. That range includes FDA rejection risk, label uncertainty and other company news, so it is wider than my approval-only forecast.
Result needed for a big moveWhat the outcome has to be for the stock to move a lot instead of a little.
JAZZ needs a broad label covering both Ziihera plus chemotherapy and Ziihera plus tislelizumab and chemotherapy. Approval regardless of PD-L1 status would strengthen the commercial case. Clear launch timing and no unexpected safety restrictions would also be required for a gain above 8%.
Possible outcomesThe main ways the event could turn out, with the expected stock reaction for each one.
Positive scenario The FDA approves both combinations with broad wording and no unexpected safety restriction. Expected reaction: up 6% to 10%. The market begins valuing Ziihera as a first-line cancer franchise rather than a small biliary cancer product. Neutral scenario The FDA approves only the three-drug combination or adds restrictive label language. Expected reaction: down 2% to up 3%. Approval would be positive, but the commercial opportunity would be smaller than investors hoped. Negative scenario The FDA delays the decision or issues a Complete Response Letter because of efficacy interpretation, manufacturing or application issues. Expected reaction: down 15% to 25%. The decline could be larger if the problem requires another clinical study.
Other affected stocksOther companies that could move on the same news, such as suppliers and competitors.
ZYME, Zymeworks, is the clearest secondary beneficiary. US approval would trigger a $250 million milestone from Jazz and support future royalties. ONC, BeOne Medicines, could benefit because its drug tislelizumab is included in the three-drug regimen. MRK could face modest competitive pressure because Keytruda is used with trastuzumab and chemotherapy in parts of the first-line HER2-positive gastric cancer market. AMGN could face a small indirect risk through competition with trastuzumab biosimilar-based regimens.
What the market may be missingSomething we think investors have overlooked or read wrong.
The approval headline is less important than whether the FDA approves the two-drug Ziihera and chemotherapy regimen. This option would avoid requiring an added PD-1 medicine and could be simpler for some patients. The doublet produced a significant progression-free survival benefit, but overall survival had only shown a strong positive trend at the first analysis. Jazz expects the second interim overall survival analysis during the third quarter of 2026. A broad doublet label would therefore be the overlooked upside. A triplet-only label is the overlooked risk.
What else lands that dayOther news and events on the same day that could push the price around, and whether they help this trade, bury it, or work against it.
25 August 2026 is a quiet scheduled day. The Philadelphia Fed non-manufacturing survey is at 12:30 UTC, with consumer confidence, new residential sales and the Richmond Fed manufacturing survey at 14:00 UTC. None of those sets the direction of a specialty pharmaceutical stock. Zymeworks is decided by the same FDA action and will move on the same headline, usually harder, because the decision is a far larger share of what that company is worth. Reading Zymeworks as confirmation of your own view is reading the same news twice. The following day is heavier than this one: the second estimate of second quarter GDP and the PCE deflator arrive at 12:30 UTC on 26 August, and NVIDIA reports after the close, so index direction from Wednesday onwards has nothing to do with this approval. Net effect: the day is clear and this decision should be the only thing moving the stock. What to expect on the day. Expect the first move to be a clean read on the decision itself, and expect it to be smaller in percentage terms than the move in Zymeworks, because Jazz has an approved portfolio underneath it. If you are holding for a re-rating rather than for a headline pop, understand that you are holding through an inflation print and the largest earnings report of the quarter inside thirty hours.
Main risksWhat could go wrong, and what would prove this call wrong.
The trial's doublet overall survival result was not statistically significant at the first interim analysis. Approval may already be reflected in investor expectations. The FDA could approve a narrower population or require PD-L1-related wording. Launch adoption could be slowed by reimbursement, infusion capacity, competing HER2 treatments or physician preference for established trastuzumab regimens. Infusion-related reactions occurred in clinical studies, which could affect prescribing and label language. Short-term options are pricing substantial volatility, making a correct directional forecast potentially unprofitable if the move is smaller than the premium paid.
Other factorsAnything else worth knowing that the sections above do not cover.
Jazz's existing sleep and epilepsy products reduce dependence on this single FDA outcome. This limits the percentage downside compared with a small biotechnology company. The cleaner trade is a post-decision stock purchase. Buy only if the approval is broad, there are no major safety restrictions and the opening gain remains below 6%. Avoid buying after a double-digit opening jump because much of the near-term upside would already be captured.
